Take the completed form to Jefferson County High School Counselor's office for verification. Once the application is verified and approved the applicant will be notified.

Georgia’s total 2020 Census self-response rate is currently 42.1%. That makes us 34th out of all 50 states, Washington D.C., and Puerto Rico.
